Abstract

Introduction: Stroke is a clinical syndrome that occurs suddenly, progressively, there is a decrease in neurological function and even to death, so an early detection tool is needed as an effort to prevent the situation using an Early Warning System (EWS).  Nurses have a role in early detection in accordance with the National Standards for Hospital Accreditation and SOP set by the hospital but still found filled and unfilled EWS sheets. Method: This study uses quantitative method with 70 patient files as the sample taken by using purposive sampling. The measurement of this study is from the checklist on the completions of EWS Observational Sheets which then analyzed using frequency distribution in univariate. Presentation of data with descriptive statistics. Results: The filling of the EWS observation sheet by nurses at Bethesda Hospital Yogyakarta was filled with 36 medical records or 51.4% while incomplete amounted to 32 medical records or 45.7%. Conclusion: The filling of the EWS observation sheet by the nurse is complete although the differences with the incomplete have not been significant. Suggestion: Researchers should try to learn about what are the factors affecting the completion of EWS observational sheet by nurses.
