Abstract

Assessment of consciousness is important to estimate the prognosis of the patient's deteriorating condition. Many patients are intubated on ventilator, so an appropriate level of consciousness measurement is needed. The Full Outline of Unresponsiveness Score (FOUR) Score is a measuring tool that can assess the patient's level of consciousness. It has four assessment components, namely: brainstem reflexes, assessment of eyes, motor responses and assessment of abnormal breathing pattern. This study aims to assess the accuracy of using the FOUR score in assessing the level of consciousness of patients in the ICU. The research method is a literature review using 129 selected journals and screening according to the inclusion criteria and getting 6 journals. The results show that the FOUR Score is an excellent instrument for assessing conscious, unconscious and intubated patients. The reliability of the FOUR score is better so that the FOUR score can be an alternative or complementary to the GCS. The four components contained in the FOUR score provide detailed neurological information such as brainstem reflexes and eye movements. The FOUR score is simpler in providing better information, especially in intubated patients, so it is recommended for use in the intensive care unit. The FOUR score can be used to assess the condition of critically ill patients, both conscious and unconscious as well as intubated patients. The FOUR Score can be an alternative for assessing the level of consciousness and prognosis of intubated patients.
